Please enter the information outlined below and then select "Print Form." The system will automatically generate an account set-up form, which needs to be signed by the person responsible for your school's account and a school administrator.

Please fax the signed form to Tanja Easson at (800) 967-4027.  Once your order has been placed, your signed form has been received, and your licensing fees (if any) have been paid, your school account will be activated and the access information will be emailed to the appropriate contact person.
